Request Leave of Absence case
- On the My Information > My Calendar page, select New Request > Leave of Absence case.
- Enter or select a Start Date from the calendar .
- Enter or select a End Date from the calendar .
- Select a Category from the drop-down.
- Select a Reason from the drop-down.
- Select a Frequency from the drop-down.
- Select Submit.
- Select the leave types for the case and then select Submit.
